I have a 44 yo male manager who has been trying to conceive with his partner for approximately 5 years. This patient presents with a history of low sperm motility and >95% abnormal sperm morphology. Multiple semen analyses (11/08, 8/09) revealed normal sperm concentration/total motile sperm concentration, but low motility and abnormal sperm morphology (one assay revealed ~50% anencephalic and ~50% microcephalic). The membrane structure and function testing results were also abnormal with a sperm viability-live score of 1% (nl is >50%) and a hypoosmolarity score of 15% (nl is > 55%). An electron microscopic analysis (6/09) revealed >95% morphologically abnormal sperm. The analysis noted significant microcephaly and other "diverse widespread abnormal morphological patterns inconsistent with normal sperm function". Less than 2% of the sperm analyzed were normal. No physical genital abnormalities have been found. All hormone testing was within normal limits. Other health history - currently taking medication for hypertension (still waiting for patient to supply the name of medication - per MD, no effect on sperm quality, this sperm issue existed before taking the medication); L5 discectomy in 2004; history of taking medication to improve motility did improve motility, but had no effect on morphology/improving normal sperm numbers (as would be expected). (This patient speaks English as a second language, so obtaining the correct information verbally can be challenging.)

TCM diagnosis: Liver Damp Heat, Liver Yang rising, Kidney Yin/Essence Deficiency.
