A little discussion for us.

I have been treating a patient and all her symptoms are improving except her head 'zaps'. They start at the bottom of her occiput and go directly through her brain to yin tang area. They happen a few times at night when beginning to relax in bed.

General Patterns - Liver blood/Kidney Yin deficiency with spleen Qi deficiency and Liver yang rising.

Treated her with a modification of Tian Ma Gou Teng Yin with success of all her symptoms except this head zap thing.

This is where we could split in terms of TCM and western diagnosis, no? I have to admit that my anatomy skills aren't that great, but there is maybe/probably a vertebral artery issue or even some nerve involvement. When she is relaxing in the new position, the muscles let go and it could be showing the symptoms that she doesn't see during the day. Basically, it's the anatomical cause for Qi & Blood stagnation. Looking at the area affected it sounds like the Bladder channel has an issue.
Check for a history of neck injury (whiplash), broken bones, hernias, surgeries involving the neck area or inguinal region. Stress and other neck tightness issues (crappy posture) should be looked at as well.
Check out her:
SCM muscle (if tight treat with SJ5 & GB41 or SJ8 & GB 40)
base of occiput (painful treat with lateral to Bl40, Bl58 & Bl60)
Some other nice points are Lu8, Ren15, H3 and I remember the Dong style points for some neck issues as the points directly on the achilles tendon (between Bl60 & K3) ans well as 2" above it.
It's hard to do without more info, but that's as good a place as any to start with.
