intramural fibroids in pregnancy

I have a patient with a history of fibroids and cold womb. She has successfully shrunk fibroids with herbs in the past, but is now 7 weeks pregnant and ultrasound shows a 10cm x 8.5 cm in the muscles of the uterus. Does anyone know a good approach to 1. support and secure the fetus and 2. shrink the fibroid? Acupoints and herb formulas would be appreciated. Would moxa or TDP be indicated? Her pulse is tight and choppy, tongue is pink, scalloped, with heat speckles in the tip. She has some LBP and breast tenderness, but no t much nausea. Her uterus feels like she's 14 weeks pregnant.

I'm a bit timid moving blood in pregnancy, especially in the first trimester, but I know other practitioners who are not, and who have had great success with that. That said, I have had patients take Gui Zhi Fu Ling Wan successfully in pregnancy to resolve painful ovarian cysts. If that seems to fit her presentation, it would be a good idea.

Treat her pattern. Make sure her fibroids are from blood stasis before you (gently) move her blood. I have a lot of patients with fibroids that are due to a qi deficiency. Ai Ye if cold womb will calm fetus and warm the womb.

I have only seen three women presenting in pregnancy with this. I was using only acupuncture and working on unblocking the Ren Mai with Lu 7 and Kid 6 in addition to treating their underlying pattern – all responded well with scans showing the fibroids reduced – too small a sample to really know if this is the best approach but I found it helpful. The recommendation can be found in Giovanni’s obstetric text to use this for clearing abdominal masses and I see no problems in using these points during pregnancy …
