消 渴 小 便 利 淋 病 脈 證 并 治 第 十 三

13. On pulse syndrome complex, and treatment of Wasting thirst, Stranguria and Urinary Disturbances.

厥 陰 之 為 病 ， 消 渴 氣 上 衝 心 ， 心 中 疼 熱 ， 飢 而 不 欲 食 ， 食 即 吐 ， 下 之 不 肯 止。
　 　 寸 口 脈 浮 而 遲 ， 浮 即 為 虛 ， 遲 即 為 勞 ； 虛 則 衛 氣 不 足 ， 勞 則 營 氣 竭 。

In reverting yin disease there is wasting thirst (diabetes), surging Qi (gas) in the epigastrium with heat and pain, hunger with no desire for food, vomiting immediately after eating and incessant diarrhea following treatment with purgation. The inch pulse is floating and slow. Floating indicates vacuity and the slowness shows taxation. Vacuity will lead to Defensive (wei) Qi insufficiency, and taxation will lead to the exhaustion of Nutritive (ying) Qi.

跗 陽 脈 浮 而 數 ， 浮 即 為 氣 ， 數 即 消 穀 而 大 堅 一 作 緊 ， 氣 盛 則 溲 數 ， 溲 數 即 堅 ， 堅 數 相 搏 ， 即 為 消 渴 。

When the instep (tarsal) pulse is floating and rapid, the floating indicates the state of Qi and the rapidness indicates the state of digestion and constipation. Qi exuberance will cause frequent urination, which in turn will cause constipation. This mutual battle will then result in wasting thirst (diabetes).

消 渴 ， 小 便 反 多 ， 以 飲 一 斗 ， 小 便 一 斗 ， 腎 氣 丸 主 之 。

Wasting thirst with copious urination equal to the amount drunk, Shen Qi Wan governs.

方 見 腳 氣 中 。

　 　 脈 浮 ， 小 便 不 利 ， 微 熱 消 渴 者 ， 宜 利 小 便 發 汗 ，五 苓 散 主 之 。

When observed in the middle jiao, the pulse is floating, urination is inhibited, there is slight heat and wasting thirst. It is appropriate to disinhibit the urine, and effuse sweat. Wu ling san governs.

方 見 上 。

　 　 渴 欲 飲 水 ， 水 入 則 吐 者 ， 名 曰 水 逆 ， 五 苓 散 主 之。

When observed in the upper jiao, there is thirst with desire to drink, and vomiting when water is ingested. This is called water reversal. Wu Ling San governs.

方 見 上

　 　 渴 欲 飲 水 不 止 者 ， 文 蛤 散 主 之 。

When observed in the upper jiao, there is incessant desire to drink fluids. Wen Ge San governs.

文 蛤 散 方 ：

　 　 文 蛤 五 兩
　 　 上 一 味 ， 杵 為 散 ， 以 沸 湯 五 合 ， 和 服 方 寸 匕 。
Wen Ge San:

Wen Ge 5 liang.
Pound the above ingredient to a powder, add a square inch spoons' worth into 5 ge of water, and take warm.

淋 之 為 病 ， 小 便 如 粟 狀 ， 小 腹 弦 急 ， 痛 引 臍 中 。

In urinary strangury disease, the urine drips like millet grains. The lower abdomen is tight with pain around the navel.

跗 陽 脈 數 ， 胃 中 有 熱 ， 即 消 穀 引 食 ， 大 便 必 堅 ，小 便 即 數 。

When the fu (tarsal) pulse is rapid, this indicates stomach heat, with rapid hungering immediately after eating, constipation, and excessive urination.

淋 家 不 可 發 汗 ， 發 汗 則 必 便 血 。

In patients with stranguria it is unadvisable to promote sweating。 If sweating is promoted it will result in hematuria.

小 便 不 利 者 ， 有 水 氣 ， 其 人 若 渴 ， 栝 蔞 瞿 麥 丸 主 之 。

　 　 栝 蔞 瞿 麥 丸 方 ：

　 　 栝 蔞 根 二 兩 　 茯 苓 三 兩 　 薯 蕷 三 兩 　 附 子 一 枚 （ 炮） 　 瞿 麥 一 兩
　 　 上 五 味 ， 末 之 ， 煉 蜜 丸 梧 子 大 ， 飲 服 三 丸 ， 日 三 服 ； 不 知 。 增 至 七 八 丸 ， 以 小 便 利 ， 腹 中 溫 為 知 。

When urination is inhibited there is edema. If the patient is thirsty, Gua Lou Qu Mai wan governs.
Formula: Gua Lou Gen (Tian Hua Fen) 2 liang Qu Mai 1 liang
Fu Ling 3 liang
Shu Yu 3 liang
Fu Zi 1 piece (blast fried)

Powder the above 5 ingredients, condense with honey and form into pills the size of Wu Zi (Sm. Sterculia).
Take 3 pills with water, 3 times a day. If still no effect, increase the dosage to 7 or 8 pills, until the urine is disinhibited, and the abdomen feels warm.

小 便 不 利 ， 蒲 灰 散 主 之 ； 滑 石 白 魚 散 ， 茯 苓 戎 鹽 湯 并 主 之 。
蒲 灰 散 方 ：

　 　 蒲 灰 七 分 　 滑 石 三 分
　 　 上 二 味 ， 杵 為 散 ， 飲 服 方 寸 匕 ， 日 三 服 。

When urination is inhibited Pu Hui San governs, as do Hua Shi Bai Yu San and Fu Ling Rong Yan Tang.
Pu Hui San:
Pu Hui 7 fen Hua Shi 3 fen
Pound the above ingredients to a powder, and take a square inch spoons worth with water 3 times a day.

滑 石 白 魚 散 方 ：

　 　 滑 石 二 分 　 亂 髮 二 分 〔 燒 〕 　 白 魚 三 分
　 　 上 三 味 ， 杵 為 散 ， 飲 服 方 寸 匕 ， 日 三 服 。

Hua Shi Bai Yu San:
Hua Shi 2 fen Luan Fa 2 fen (burnt) (human hair)
Bai Yu 3 fen
Pound the above ingredients to a powder, and take a square inch spoons worth with water 3 times a day.

茯 苓 戎 鹽 湯 方 ：

　 　 茯 苓 半 斤 　 白 朮 二 兩 　 戎 鹽 彈 丸 大 一 枚
　 　 上 三 味
Fu Ling Rong Yan Tang: Fu Ling 0.5 jin Bai Zhu 2 liang
Rong Yan (salt crystal) 1 piece the size of a bullet.

渴 欲 飲 水 ， 口 乾 舌 燥 者 ， 白 虎 加 人 參 湯 主 之

When there is thirst with a desire to drink, dry mouth and tongue, Bai Hu Jia Ren Shen Tang governs.

　 脈 浮 發 熱 ， 渴 欲 飲 水 ， 小 便 不 利 者 ， 豬 苓 湯 主 之。

　 　 豬 苓 湯 方 ：

　 　 豬 苓 （ 去 皮 ） 　 茯 苓 　 阿 膠 　 滑 石 　 澤 瀉 各 一 兩
　 　 上 五 味 ， 以 水 四 升 ， 先 煮 四 味 ， 取 二 升 ， 去 滓 ,內 膠 烊 消 ， 溫 服 七 合 ， 日 三 服 。

When there is a floating pulse, heat effusion (fever), thirst with a desire to drink, and inhibited urination, Zhu Ling Tang governs.

Zhu Ling Tang: Zhu Ling (remove skin) Fu Ling Hua Shi Ze Xie E Jiao 1 liang each.
In 4 sheng of water, boil the first 4 ingredients until 2 sheng remain. Remove the dregs. Melt the E Jiao in the decoction. Take 7 ge warm, 3 times a day。
